Request for Fire Department Incident Report

  1. Pursuant to the Georgia Open Records Act (OCGA 50-18-71 et seq.) and the City of South Fulton Records Policy, the Community Risk Reduction Office provides copies of City public records.
     
     To obtain a copy of a Fire Department Incident Report public record, please complete the following form.

  4. I agree to pay any copying and/or administrative costs incurred in fulfilling my request to the extent permitted by Georgia law. Such costs shall include a $5.00 report fee and may include copying charges of $.10 per page and administrative charges for search, retrieval, and other direct administrative costs. Administrative charges shall not to exceed the salary of the lowest-paid full-time employee who, in the discretion of the custodian of the records, has the necessary skill and training to perform the request. There is no cost for the first fifteen minutes needed to complete this request.
