Citizen Complaint Form

  1. Please note:
    • Complaints concerning employees or services provided by the Sheriff's Office, Courts, Health Departments, Tax Commissioner or Extension Service should be directed to those agencies utilizing their complaint procedures.
    • ADA/TITLE VI complaints must be submitted within 180 days of occurrence. 
    • Once we have received your complaint we will respond to you via email or phone within 10 days.
    • Our investigation into your complaint may include internal investigations or external agencies.
